Sacramental Record Request

If you or your child received a sacrament at Holy Trinity Newman Center and you are in need of a copy of the sacramental record, the request must be in writing (download record release form here). Please email the filled out form to [email protected] and we will mail the certificate to the church in need of the information. If you provide a scan of a valid ID we can send the certificate directly to you. 

Certificates are only issued to the parent of a child (or an individual who presents proof of guardianship, power of attorney or executor, personal representative or other legal status) or to the person to whom the record is referring. 
Sacramental records are not open for genealogical purposes or family histories.
